The goal of this program is to increase awareness and knowledge of the role of social determinants of health among healthcare professionals in order to improve clinical outcomes as well as patient and staff satisfaction. The conference will involve an introduction to social determinants of health as well as 30-minute presentations given by faculty from physical therapy, pharmacy, dentistry, social work and nursing. Following presentations, volunteers will participate in a live simulation of a case conference that includes an interdisciplinary team meeting where the patient is included in the plan of care. The goal of the simulation is to examine how implicit biases of healthcare professionals can impact patient care as well as to encourage all healthcare professionals to work toward a shared understanding of a problem and enhance communication across the healthcare disciplines. The simulation will be live streamed to the rest of the attendees and will be followed by debriefing and small group discussions.
